• There is a $25 application fee per adult.

• Please make sure the application is completely filled out.
• Once the application is complete, fax it to (951) 691-5426.

Speed up the Process:

Fax in your last two pay stubs and bank statements with account numbers crossed out along with your application.

Self Employed: Fax copies of your last two years 1099 or W2's and profit and loss.
